At Cancer Copay Relief we have come up with two ways someone can be connect to us.  

1. The first is that you yourself are an individual that is in need of copayment assistance.  In this scenario, we have setup an application process that will walk you through the few simple steps, and answer any questions you may have.

2. The second is a referal program, where you can refer a loved one or friend.  We have customized a referal form that you can fill out and even notify your friend or loved one with a personal message.  This form is equally as simple and user friendly as our normal application form, but designed to reach out to those people who love someone that may be in need of help.
